This tool was developed by TMF to help physicians and ancillary personnel provide effective diabetes mellitus care management. The flow sheet can be strategically located in a patient’s chart and used to track on-going treatment. The flow sheet is based on U.S. Preventive Services Task Force, American Cancer Society, HEDIS Measure year 2000 and the National Osteoporosis Foundation guidelines. |
